The Lockout/Tagout Authorized Person(s) Periodic Review (Word) is used to inspect the Authorized Person’s role and responsibility established in the hazardous energy control program and confirm that the Authorized Person has sufficient knowledge of hazardous energy control procedures and their application to lockout/tagout and tagout only.
The LOTO Lock Removal Form (Word) includes directions and specific information a department or unit must document when an Authorized Person (lock owner) is unable or unavailable to remove their personal lock during a LOTO procedure.

For equipment that is routinely maintained and serviced, requires a full shutdown for servicing, and has good change management practices in place, the UW LOTO Procedure form can be used.

In some cases, permit-required confined spaces can be reclassified as non-permit spaces. Download the guidance document below and follow the instructions to temporarily reclassify a permit required confined space.
If you suspect a space meets the definition of a permit-required confined space, but you do not have a permit to enter, do not enter the space. Complete the Confined Space Evaluation form (below) and submit it to EH&S for evaluation.

Units and departments can use the Walking-Working Surfaces Inspection Checklist to identify areas or conditions for potential slip and trip hazards, especially in office and general work areas.
